Paperwork (Forms)

As a reminder, every team must perform final check in. Our official check in night is scheduled for Thursday May 21st from 6-9 PM at Dick's Sporting Goods in Schaumburg (601 N. Martingale).

We strongly encourage each team to check in on Thursday night but if its not possible, you can check in at the Olympic Park main tent no later than 1 hour before your first game. We do not perform check in at Connnelly Park or Palatine HIgh School. 

Please note that even if all your paperwork is in, you still need to check in to have your player passes certified and banded.

 



2015 Grove United Memorial Day Shootout

Team Paperwork (due by May 8, 2015)

 

A reminder that team paperwork for this year’s Grove United Memorial Day Shootout is due by Friday, May 8, 2015.  Below is an explanation of the paperwork and how you can get it to us.

 

Paperwork can be submitted in the following ways:

Fax copies of your documents will not be accepted.

 

Electronic options:

Please upload forms as a pdf file (no jpg submissions will be accepted).  File names should include the Club/team name, gender, age group, and type of document (e.g. Tournament Roster, Medical Releases, …).  Medical Release forms should be uploaded as one file, rather than individually scanned and uploaded as separate files.

Required documents:

IYSA affiliated teams (IWSL, YSSL, CIYSL, SISL, Illowa) submit:

  1. A copy of your IYSA Tournament Roster Form (either the form on our tournament site, the IYSA site, or your team page in your league’s site).  All players, including guest players, must be listed on the teams IYSA Sanctioned Tournament Roster.
  2. A copy of your League certified roster (IWSL, YSSL, CIYSL, …)

Note: You do not need to submit IYSA Medical Release forms.  You are required to have them at all games, and will need to provide a copy for any player receiving medical treatment.

 

US Club teams (NISL, out-of-state US Club teams) submit:

  1. A copy of your IYSA Tournament Roster Form (either the form on our tournament site or the IYSA site).  All players, including guest players, must be listed on the teams IYSA Sanctioned Tournament Roster.
  2. A copy of your US Club certified roster (NISL, …)
  3. IYSA Medical Release forms for all players (US Club Medical Release forms cannot be accepted)
  4. Form NT
  5. IYSA Youth Program Background Check and Concussion Awareness Certification Form

 

Out of State USYSA affiliated teams submit:

  1. A copy of your IYSA Tournament Roster Form (either the form on our tournament site or the IYSA site).  All players, including guest players, must be listed on the teams IYSA Sanctioned Tournament Roster.
  2. A copy of your State certified roster (from the league you are playing in).
  3. IYSA Medical Release forms for all players (out-of-state Medical Release forms cannot be accepted)
  4. An approved “Permission to Travel” form from your home state’s USYSA affiliate.
  5. IYSA Youth Program Background Check and Concussion Awareness Certification Form

 

Guest Players

  • A team can have up to 5 guest players.
  • All players on a team must have player passes from the same soccer affiliation.  IYSA affiliated leagues include IWSL, YSSL, CIYSL, SISL, and ILLOWA.   US Club affiliated local leagues include NISL and ICL.  IYSA and US Club are not affiliated.
  • A player with a player pass from the same club and the same soccer affiliation is not considered a guest player.
  • Guest player forms are not required for players from outside the borrowing club.  Possession of the player pass is sufficient.  Note: the guest player pass must be from the same soccer affiliation.
  • All players, including guest players, must be listed on the teams IYSA Sanctioned Tournament Roster.

 

INSTRUCTIONS FOR UPLOADING TEAM DOCUMENTS THROUGH GOTSOCCER

  1. In order for your team files to be accepted electronically, you must include the following in the file name:  Club name, team name, age group you will be playing in, team gender, and what the content of the document is.  File’s need to be scanned or downloaded from your league’s website as PDF formatted documents (no .jpg files).  Multi-page documents (e.g. IYSA Medical Release forms) need to be uploaded as a single file.

  1. Log into your team’s GotSoccer account
  2. You should open to your home page showing your “Event Registration History”.  Choose “Grove United Memorial Day Shootout – Presented by Quaker Oats” by clicking on the name of the tournament.

 

  1. You should now be at the “My Events” page.  If not, click on “Events” towards the top of the page (right next to “Home”).  In the body of the page, there will be multiple tabs.  Click on the tab named “Documents” on the far right side.

 

  1. You should now be at the page to upload documents (see below).  Click in the box named “File Name/Description” and put the name of the file using the format described in step 1.

  1. Click the “Browse” to locate the file on your hard drive.  Double click on the file to enter the path name.  Then click on “Upload File”.
  2. Repeat this step for each of the documents you need to submit.

 

  1. The documents you uploaded should be viewable as shown above.  Once we have reviewed your documents, the box titled “Verified” will go from  to   (see below).  You can log back into GotSoccer at any time to check the status of your documents.
