Memorial Day Shootout Registration and Check-in
Registration documents
Please mail or fax copies of the following items by May 11:
1) IYSA Sanctioned Tournament Roster.
All teams must provide a completed IYSA Sanctioned Tournament Roster Form. The tournament roster may be changed up to final check in. At that point it is final and cannot be altered for the tournament.
2) IYSA Medical Release/Liability Waiver for each player.
All teams, including US Club teams, must submit an IYSA Medical Release and Liability Waiver Form for each player. (copies must be left at registration).
3) Certified State roster (should have the league’s stamp on the roster)
All teams must provide a legible copy of their approved State USYSA roster with state add/drop forms (as needed) or US Club Rosters (copy must be left at registration)
4) Certified Add/Drop form for new players to your roster (as needed).
5) Tournament Guest Player Forms (as needed).
USYSA teams with guest players must have an IYSA/USYSA tournament guest player permit form. For US Club teams, a guest player permit is not necessary, however players must be on your official US Club Roster.
6) Permission to Travel Form.
USYSA teams from outside Illinois must present an approved “Permission to Travel” form from their home state’s USYSA affiliate (copy must be left at registration). US Club teams are not required to have a travel permit.

Check-in:
Teams should plan on attending check-in on Thursday, May 21st from 6-9 p.m. at Soccer Madness (located at 364 W Half Day Rd in Buffalo Grove ).
Please note that we are expecting two members of the Chicago Red Stars, our Womens Professional Soccer team, to be at Soccer Madness from 6:30-8:00 PM signing autographs. Bring your kids!
In addition, all teams registering Thursday night can enter, free of charge, a raffle valued at $800 in soccer items provided by Soccer Madness. In order to make sure that check-in goes smoothly and quickly we ask that you attend on Thursday evening to ease congestion on game days. However, if you cannot attend that evening, you may check in at the Tournament Headquarters (Olympic Park, 1675 E. Old Schaumburg Road , Schaumburg , Illinois ) at least one hour prior to the start of your first game.
At the time of check-in, the following documents must be presented:
1) All player passes. Each player and coach must have a current/valid USYSA or US Club Player Pass.
2) Emergency contact information for the team manager and/or coach while attending the tournament. Please provide a team contact’s cell phone number. We will also be using these numbers in case of weather related delays.
