    Discounts: Bring 9 teams, the 10th is FREE 
    • Parking fees of $50 per team (U8-U10), $75 per team (U11-U19) not included
  • Mandatory Paperwork Required by May 14th (see the FORMS page for assistance)
    • Certified Team Roster with the state’s or league’s approval clearly visible. (2 copies)
    • IYSA Sanctioned Tournament Roster – signed by the coach. (2 copies)
    • IYSA Medical Release Forms (2 copies per player)
    • Tournament Team Agreement (Form NT). (required for all teams playing under US Club passes, or NON-IYSA)
    • IYSA Guest Player Form (as needed)
    • Permission to Travel Form (as needed)

   ALL TEAMS are required to attend check-in on Thursday May 27th from 6-9 pm at the Dicks' Sporting Goods

                      Located at The Streets of Woodfield, 601 North Martingale, Schaumburg, IL 60173 .  

                      If you cannot attend this, you may check in at the Tournament Headquarters at Olympic park.  

                     Olympic Park check-in must be at least one hour prior to your first game.

  AT CHECK-IN BE PREPARED TO ALSO PROVIDE:

              Player and coaches passes – Current and valid USYSA or US Club passes are required for all players and coaches.

              Emergency contact information for the coach or team manager while at the tournament.
